Instructions for the Person Taking the Test

  1. On the circle below (or on a blank piece of paper), draw a clock face.
  2. Write all 12 numbers in their correct positions around the clock face.
  3. Draw two hands on the clock showing the time 11:05 (five minutes after eleven o'clock).
  4. The minute hand should point toward the 1, and the hour hand toward the 11.
  5. Work without looking at a real clock.

How to Score the Clock Drawing

Ask a family member, caregiver, or trusted person to score your drawing using the table below. Or score it yourself as honestly as you can.

3 pointsMinor error only — e.g., one number slightly out of place, or one hand slightly off, but overall clearly correct.
2 pointsModerate errors — e.g., several numbers misplaced, hands confused (both pointing same direction), or time noticeably incorrect.
1 pointSignificant errors — e.g., numbers missing or very disorganized, only one hand drawn, clock shape abnormal.
0 pointsNo recognizable clock drawn, or nothing drawn.
